Common forms are forms which span across several study events, such as a Concomitant Medication or Adverse Event form. Common forms are associated to the entire subject casebook, rather than a specific timepoint. Veeva EDC displays common forms in the casebook schedule and the breadcrumb menu. Clicking a common form the breadcrumb menu displays a table view of any existing forms.

Depending upon study design, common forms may not appear in the EDC until specified criteria are met or a specified event occurs. More information about when common forms will appear for a study can be found in the “Folder (Visit) and Form Availability” section or within the specific form completion guidelines.

Some common forms may be designed as repeating forms. Repeating forms are forms that you can use multiple times during a study. These forms allow users to capture data for the same set of questions multiple times during a single event or study. Note that the study’s design may limit the number of times a form can repeat.

Adding a Common Form
If the common form is a single form, you can click the form name under the Common Forms heading which opens the form for editing. Complete data entry and submit the form as you would with any scheduled form.

You can add repeating common forms by navigating to the common form by clicking on the form name under the Common Forms heading or through the breadcrumb menu.

  1. From the table view in the right pane, click + New.
  2. Veeva EDC adds the new form to the subject’s casebook and opens the form in edit mode.
  3. Enter data in the form and click Submit.

To return to the repeating common form table view, click the form name at the top of the form page or by clicking on the form name under the Common Forms header.
