Entering Unknown Date Elements or Unknown Time

Whenever possible, complete dates should be reported. However, depending on the study design, you may be able to mark certain dates (either the month and day or only the day) and / or times as unknown.

Not all date / time fields allow for unknown elements to be entered. If you do not see the “Unknown” checkbox for a date field or are not able to enter “UN” or “?” in a time field, this field requires a full date or time to be entered. If you do not enter the correct date or time, a query message may appear, and the form may not be able to be submitted.

!On all forms set up as Lab forms (i.e. Blood Chemistry Lab Panel), the Collection Date Time fields require a full date and time to be entered. No partial dates or unknown time can be entered. This is by design, as most often dates and times of lab collections/tests will be known.
