A completed Form 49 is required to create new user accounts, amend accounts, reactivate, or deactivate accounts. Please ensure the following sections of the Form 49 are completed correctly: -

  1. Authorising Officer Details – Executive Committee Members or Staff Members can approve. NOTE: Applicants are not able to authorise their own Form 049.
  2. User Agreement – must be signed by the person who is having the account created/amended for them.
  3. Submit or follow your states process for the submission of signed Form 049’s
