Overview
The Research Readiness Report is a summary of a center’s reporting metrics and key performance indicators (KPIs). This report is to be used as a guide to help the center visualize reporting trends, center progress, assess gaps, and/or identify issues to be addressed.
Centers in “Active – Developing” status will receive these reports on a weekly basis (Tuesdays) instead of the CPI summary reports. All “Active – Established” and “Active – Follow-Up” centers will receive this report on an annual basis.

Run Date
Indicates the date and time (Central Time) the report was generated.
Tables
Table 1: Reporting Preferences
Table 1 indicates the center’s reporting preference. If centers need to update the reporting preferences, visit this link for further instructions.
| Reporting Type | Reporting Preference |
| Autologous HCT | |
| Allogeneic Related HCT | |
| Allogeneic Unrelated HCT | |
| Cellular Therapy |
Table 2: Count of Infusions by Donor Type
Table 2 summarizes the count of infusions by donor type for the past 2 years. Note, if Table 2 does not correlate with Table 1, centers may need to update their reporting preferences. CIBMTR expects participating centers to report on their cases regularly. Centers without activity reported for more than 2 years may be moved to inactive status.
| Donor Type | Count of Infusions | Percent of Infusions |
| Autologous HCT | ||
| Allogeneic Related HCT | ||
| Allogeneic Unrelated HCT | ||
| HCT – Donor Type Not Yet Reported | ||
| Cellular Therapy (CT) | ||
| Gene Therapy (GT) | ||

Table 3: Count of Infusions by Product Type
Table 3 summarizes the count of infusions that have been infused by product type for the past 2 years.
| Product Type | Count of Infusions |
| HCT Event – Bone Marrow (BM) | |
| HCT Event – Peripheral Blood Stem Cells (PBSC) | |
| HCT Event – Cord Blood Unit (CBU) | |
| HCT Event – Other Product | |
| CT Event | |
| CT Event – Other Product | |

Table 4: Average Days from CRID Registration to Infusion Date
Table 4 summarizes the average number of days from CRID registration to infusion date for first infusion only. CIBMTR expects CRIDS to be registered within 30 days of infusion to ensure patients are enrolled in studies in a timely manner and prevent volatility in CPI metrics.
Table 5: Patient Consent Rate by Donor Type
Table 5 summarizes the counts and rates (by donor type) of patient’s consent data to be used in research. CIBMTR’s reporting requirements suggest all consent options (besides “yes”) should be lower than 5%. If your center has a percentage higher than these standards, please inform CIBMTR of these reasons. For additional information on consent, visit this link.
| Consent | Allo HCT (Count) | Allo HCT (%) | Auto HCT (Count) | Auto HCT (%) | HCT (Donor Type Not Reported) (Count) | HCT (Donor Type Not Reported) (%) | CT (Count) | CT (%) | Total (Count) | Total (%) |
| No | ||||||||||
| Not Approached | ||||||||||
| Not Yet Reported | ||||||||||
| Yes |
Table 6: Recipient Lost to Follow-Up (LTF) Rate
Table 6 summarizes the counts and rates of recipients indicated as lost to follow up (LTF). For additional information on consent and LTF status, visit this link.
| LTF Reason | LTF Recipients (Count) | LTF Recipients (%) |

Table 7: CIBMTR Reporting
Table 7 contains 2 tables listing the records that have been reported to NMDP but not yet reported to CIBMTR. Table 7.1 indicates that a CRID assignment form (used to add a recipient in FormsNet) has not been completed or has been completed but the NMDP RID was not reported. Table 7.2 indicates that a recipient has been reported in FormsNet but the event date has not been reported. These tables will be populated if there is only one NMDP transplant center (TC) correlated with one CIBMTR Center Number (CCN).
| NMDP RID | Product Receipt Date | NMDP Planned Infusion Date | FormsNet CRID | FormsNet NMDP RID | FormsNet Infusion |
| DD-MM-YYYY | DD-MM-YYYY | No Infusion Reported |
Table 8: Critical Form Completeness
Table 8 summarizes critical form completeness metrics. The CPI program has the following requirements for critical form completeness: 1) critical forms completed on time (50% or more) and 2) critical forms completed in previous trimester (98% or more). For additional information on critical form completeness and requirements, visit this link.
| Form | Forms Completed by Due Date (%) | Forms Completed by Previous CPI Trimester (%) | Average Days to Critical Form Completeness |
| Form 2814 | |||
| Form 2400 | |||
| Form 2402 | |||
| Form 4000 | |||
| Form 2006 | |||
| Form 2807 | |||
| All Critical Forms |
Table 9: Overall Form Completeness
Table 9 summarizes overall form completeness metrics. The CPI program has the following requirements for form completeness: 1) critical forms completed on time (50% or more), 2) critical forms completed in the previous trimester (98% or more), 3) study supplemental forms completed in the previous trimester (100%), and 4) all other forms completed in the previous trimester (95% or more). For additional information on form completeness and requirements, visit this link.
| Form | Forms Completed (Count) | Forms Completed (%) | Forms Incomplete (Count) |
| Critical Forms | |||
| Study Supplemental Forms | |||
| All Other Forms | |||
| All Forms |
Table 10: Unresolved Queries
Table 10 summarizes the metrics for unresolved queries. Queries are placed when CIBMTR identifies potential errors and discrepant or missing data. The CPI program has the following requirement for query resolution: queries placed in the previous trimester (95% or more). Centers are requested to resolve queries within 6 weeks of the query placement date. For additional information on query resolution, visit the FormsNet3 Training Guide.
| Query Location | Section | Unresolved Queries (Count) | Unresolved Queries (%) | Average Days to Query Completion |
| All unresolved queries |
Table 11: Center Support Tickets
Table 11 summarizes the metrics on tickets submitted by your center to CIBMTR Center Support. For any questions or concerns, submit a ticket to CIBMTR Center Support.
| Ticket Category | Ticket Subcategory | Center Support Ticket (Count) | Center Support Ticket (%) |
