Overview
For centers in a Developing status, a milestone evaluation report will be distributed at the end of each trimester. This report provides a final summary of your center’s reporting metrics, form completion history and next steps. Centers in a Developing participation status are reviewed each trimester to determine if the threshold milestones are met to ensure complete and high-quality data are reported before being promoted to an Established participation level. This report can also be used as a tool for centers seeking FACT accreditation or aiming to becoming an Established center.
Milestones to Becoming an Established Center
- US Allogeneic center
- One trimester meeting threshold milestones (refer to table below)
- Consent rate greater than 75% (refer to Table 5 in the Research Readiness Report)
- All other centers
- Two consecutive trimesters meeting threshold milestones (refer to table below)
- Consent rate greater than 75% (refer to Table 5 in the Research Readiness Report)

Table 1 indicates if a center has met the milestones to be promoted to an Established center.
Table 2: Milestone Standings
Table 2 represents your center’s final standing for each of the milestone threshold targets for the current trimester and prior trimester (if applicable).
Table 3: Current Trimester Form Completion History Summary
Table 3 represents your center’s form completion history over the current trimester grouped into form categories (critical, study, and other forms). The table displays the forms included in the current trimester and how many forms were completed in each month of the trimester. For quality data practices, centers should complete the forms consistently throughout the trimester, rather than completing high form volumes in the last few weeks.
Table 4: Next Steps
Table 4 indicates your center’s next steps based upon if your center has met the milestones. If your center did not meet the milestones, there will be recommended training courses available. If your center did meet the milestones, your center will need to complete the survey linked to opt in to becoming an Established center.
