If the Discrepancy Report indicates a HCT date mismatch between the submitted CTA spreadsheet and the data in FormsNet3SM, the center must determine where the error(s) occurred and update as described below.

Common Issues to look for:

  • Date and Month switched
  • Reporting planned date vs actual date – FormsNet3SM should have actual date
  • Date mis-entered/auto-corrected

If the submitted CTA spreadsheet was incorrect:

  1. Update the value(s) on the spreadsheet
  2. Resubmit the spreadsheet via CIBMTR Center Support (https://nmdp.service-now.com/csm).
    1. Select CTA and CTA list resubmission

If FormsNet3SM is incorrect:

For the first transplant:

  1. From the FormsNet3SM Recipient Tab, enter the CRID you want to edit and Click Search
  2. Click on the Edit Form icon for the form 2814
  3. Correct the “planned HCT date” to be the actual date the HCT started.
  4. Click Submit to rerun form validation. Resolve any errors or queries that are generated.
  5. Click Continue with Submission. A confirmation will display Form Status: Complete when all issues have been addressed.

Repeat this process for each completed form with this infusion date.

To Correct HCT Date for a subsequent transplant:

  1. Locate the form where the subsequent infusion was reported – it is usually the last follow-up form completed for the prior transplant.
  2. Click on the Edit Form icon for the form that reported the subsequent infusion
  3. Collapse the sections. Scroll to the Subsequent HCT / Transplant / Infusion section to expand.
  4. Update the Date of Subsequent HCT.
  5. Click Submit to rerun form validation. Resolve any errors or queries that are generated.
  6. Click Continue with Submission. A confirmation will display Form Status: Complete when all issues have been addressed.

Repeat this process for each completed form with this infusion date.
