Is this form required for all CRIDs?
Yes, this form is mandatory for all CRIDs created after October 24th, 2025.
For existing CRIDs created prior to October 24th 2025, F2804 (CIBMTR Research ID Assignment) will provide view-only fields of ethnicity and race. If an update to that data needs to be made,F2807 will need to be created on-demand and completed. Once REA data exists for the recipient, the view-only fields on F2804 will no longer display.
What if a center is prohibited from collecting/reporting race, ethnicity and ancestry information?
