Discrepancy Type Description and Action Item
DOB (Date of Birth) Mismatch The patient’s date of birth on CTA list is discrepant with FormsNet3 data.
  • If the date on the CTA list is incorrect, centers must correct their CTA list and resubmit it via CIBMTR Center Support ticket. Select CTA > CTA List Re-Submission for Discrepancies as ticket category
  • If the date in FormsNet3 is incorrect, centers must correct FormsNet3 by updating the CRID Assignment (2814) Form and reprocessing forms that contain the recipient’s DOB.
    • Navigate to the CRID assignment form for the CRID in question by following the Search for a CRID guide
    • Edit the CRID Assignment (2814) Form by following the Editing the CRID Assignment guide.
      • Correct the recipient’s date of birth
    • Reprocess all other forms that contain the recipient’s DOB
      • Open each form via the edit icon > allow auto-population to update recipient’s DOB > submit form to complete
      • Open form with edit icon > manually correct recipient’s DOB > submit form to complete
Note: Recipient forms include F2400, 2000 (r3) and all F2450s (except r4). Error corrections may also be required for non-FormsNet3 editable forms
Sex Mismatch The patient’s sex on CTA list is discrepant with FormsNet3 data. *If the CTA list is incorrect, centers must correct their CTA list and resubmit it via CIBMTR Center Support ticket. Select CTA > CTA List Re-Submission for Discrepancies as ticket category
  • If a FormsNet3 is incorrect, centers must correct FormsNet3 by updating the CRID Assignment (2814) Form and reprocessing forms that contain the recipient’s Sex.
    • Navigate to the CRID assignment form for the CRID in question by following these steps
    • Edit the CRID assignment form by following these steps
      • Correct the recipient’s Sex
    • Reprocess recipient forms that contain the recipient’s Sex
      • Open form with edit icon > allow auto-population to update recipient’s DOB > submit form to complete
      • Open form with edit icon > manually correct recipient’s DOB > submit form to complete
Note: Recipient forms include F2400, 2000 (r3) and all F2450s (except r4). Error corrections may also be required for non-FormsNet3 editable forms
HCT Date Mismatch The transplant date on CTA list is discrepant with FormsNet3 data.
  • If the CTA list has an error, centers need to make corrections to their CTA list and resubmit their CTA list via CIBMTR Center Support ticket. Select CTA > CTA List Re-Submission for Discrepancies as ticket category
  • If a FormsNet3 error is noted, centers must make correction in FormsNet3
    • In FormsNet3 Recipient Forms page, enter the CRID in the search box & click the Search button
    • Correct the HCT Date
      • If first transplant reported in FormsNet: click the Edit icon for form 2814 > update the event date > submit the form to complete. Resubmit all other recipient forms that are not in DUE status to update the event date within the form.
      • If subsequent transplant: click the Edit icon for the follow up form (2450, 2100, 4100) that reports the subsequent transplant > update the event date > submit the form to complete. Resubmit all other recipient forms that are not in DUE status to update the event date within the form.
NOTE: If a subsequent transplant is reported in a form 2814, create CIBMTR Center Support ticket to resolve.
HCT Type Mismatch The HCT type on CTA list discrepant with FormNet data.
  • If the CTA list has an error, centers need to make corrections to their CTA list and resubmit their CTA list via CIBMTR Center Support ticket. Select CTA > CTA List Re-Submission for Discrepancies as ticket category
  • If a FormsNet3 error is noted, centers must make correction in FormsNet3
    • In FormsNet3 Recipient Forms page, enter the CRID in the search box & click the Search button
    • Click on Edit icon for the Form 2400
    • Scroll to Donor Information section
      • NOTE: There may be more than one donor and more than one donor instance in this section. Ensure all instances are correctly reporting their donor type
    • Correct HCT type (often referred to as donor type within the form) > click Save Now > submit form to complete
    • Reprocess additional recipient forms for the donor that were completed (Form 2004, 2005, and/or 2006). The visit details on the FormsNet3 forms grid will often display the donor used
      • Open form with edit icon > allow auto-population to update recipient’s donor type > submit form to complete
      • Open form with edit icon > manually correct recipient’s donor type > submit form to complete
HCT Date Missing CIBMTR does not have a record of the HCT reported on the CTA list. For these cases, a CRID exists in FormsNet3
  • Register transplant in FormsNet3.
    • In FormsNet3 Recipient Forms page, enter the CRID in the search box & click the Search button
    • Report the HCT
      • If first transplant reported in FormsNet: click the Edit icon for form 2814 > update the event date > submit the form to complete.
      • If subsequent transplant: click the Edit icon for the follow up form (2450, 2100, 4100) that reports the subsequent transplant > update the event date > submit the form to complete.
NOTE: A subsequent transplant should ONLY be reported in a Form 2814 if no follow up forms exist for a prior event.
Missing CRID CIBMTR does not have a record of the CRID on CTA list.
  • Confirm correct CRID is reported on the CTA list. If the CTA list has an error, centers need to make corrections to their CTA list and resubmit their CTA list via CIBMTR Center Support ticket. Select CTA > CTA List Re-Submission for Discrepancies as ticket category
  • If patient has yet to be registered in FormsNet3, then add the CRID and transplant in FormsNet3.
    • To register the patient, complete a CRID assignment form. This can be completed by following the Create a CRID guide. NOTE: If this is not the patient’s first cellular therapy, they may a CRID within FormsNet. Search in the Search/Edit CRID tool to determine if a CRID already exists for patient.
    • To register an HCT event, follow the steps in the HCT Date Missing section

RID Not Reported in FormsNet3 The patient’s NMDP RID is not reported in FormsNet3.
  • This could be due to:
    • NMDP RID blank in the CRID Assignment Tool.
    • The NMDP RID in the CRID Assignment Tool is incorrect.
    • A CRID has not yet been assigned in FormsNet3
  • Add the NMDP RID in FormsNet3
    • Navigate to the CRID assignment tool for the CRID in question by following the Search for a CRID guide
    • Edit the CRID assignment form by following the Editing a CRID Assignment guide.
      • Add the recipient’s NMDP RID
HCT Not Reported (between NMDP & FormsNet3) NMDP has a record a product was provided to a center. However, there is not an infusion date in FormNet3 for that NMDP RID near (within 10 days) of the infusion date provided to NMDP. The infusion may exist in FormsNet but the infusion date reported in FormsNet and NMDP is more than 10 days apart.
  • If the transplant did not occur, submit a Center Support ticket to inform CIBMTR. CIBMTR will inform NMDP to remove infusion.
  • If the transplant occurred, but the date does not match the date provided to NMDP, submit a Center Support ticket to inform CIBMTR. CIBMTR will inform NMDP to remove infusion.
  • If HCT did occur, register CRID and/or transplant in FormsNet3.
    • To report the CRID, follow the steps in the Missing CRID section
    • To report the event, follow the steps in the HCT Date Missing section
