Comprehensive Report Form (CRF) Center

A transplant center designated as CRF level will submit either Transplant Essential Data (TED) forms or Comprehensive Report Forms (CRF). The type of follow-up forms a CRF center uses for a specific recipient is determined by the CIBMTR’s selection algorithm. If the recipient does not consent to participate in CIBMTR observational research, then HCT follow-up data must be submitted on the Post-TED forms for allogeneic recipients. Recipients participating in certain clinical trials may be required to submit CRF level data, even if the recipient declines consent to the observational database.
