2807: Race, Ethnicity and Ancestry (REA)

The Race, Ethnicity and Ancestry (2807) Form collects recipient race, ethnicity, and ancestry (REA) data and is required to be completed for all recipients, regardless of if the recipient is not a US resident and the recipient’s country’s rules / regulations prohibit the collection and / or reporting of race, ethnicity or ancestry data. This form will come due after completing and submitting the CIBMTR Research ID Assignment (2804) form.

Previously Reported Recipients to CIBMTR
For recipients already existing in FormsNet3^SM (i.e., the recipient had a prior infusion already submitted to CIBMTR), the Race, Ethnicity and Ancestry (2807) will not come due as the recipient’s demographics had been previously reported; however, this form is available on-demand. If the recipient’s REA data needs to be updated, the Race, Ethnicity and Ancestry (2807) Form must be made due on-demand to update the information and submit the form.

Transferred Recipients

• Previously Reported Recipients
  • For recipients without a previously completed Race, Ethnicity and Ancestry (2807) Form who are transferred to another center and the REA data needs to be updated, the Race, Ethnicity and Ancestry (2807) Form must be made due on-demand to revise the REA information and submit the form. The former center will not be able to edit the form but can still view the data.
• New Recipients
  • When a recipient with a previously completed Race, Ethnicity and Ancestry (2807) is transferred to another center, the previously completed form will transfer to the new center. The new center will be the only center who can edit the form; the former center will not be able to edit the form but can still view the data.
