All errors assigned as a Missing Documentation error require additional follow up by the auditor and/or site. If missing documentation cannot be located while the auditors are on-site, a missing documentation checklist will be sent to the site following the audit. The site will have approximately one week to provide documentation or confirm that documentation cannot be located. If data cannot be verified from a source document, the missing documentation items will be included in the site’s Corrective Action Plan. At that time, if the source documentation still cannot be located, the data will be removed from the FormsNet database after the Corrective Action Plan, signed by the medical director, is returned to the CIBMTR.
